"Traveling Clinic"

TalkTools^®/ITI / Innovative Therapists International is pleased to announce a new service provided by Sara Rosenfeld-Johnson & Associates for families and therapists. A "Traveling Clinic" can be established in your area to provide evaluations and program plan updates in your area. Here's how it works:

• We require confirmed commitments from a minimum of 6 families in any given area;
• Once the confirmations are received, scheduling will be based upon the availability of the therapist and the families;
• After agreeing on convenient dates, appointment times are scheduled.

How It Woirks:

A typical evaluation takes between ninety minutes and two hours, while the program plan update for families who have already had an initial evaluation takes an hour. You will be in the room for these sessions, and if you are working with a therapist we recommend inviting him or her to attend as well. At the conclusion of our work with your child, we will offer impressions and suggestions as if he or she were one of our regularly-scheduled clients. We suggest that you bring a video camera to record the evaluation, as no formal evaluation report will be written at that time. Instead, upon our return to the clinic in Arizona, we will write up an extensive program plan which will give you step-by-step instructions for each recommended exercise; you will receive this plan in the mail, approximately two-to-four weeks after your appointment. As you work through the program plan, any questions you or your therapist may have can be addressed to the therapist. Additionally, TalkTools^®/ITI products and tools may be recommended for implementation of this plan, some of which may be provided during the evaluation. However, the costs of the additional supplies you may need to complete the program are not included in the fee for the evaluation or program plan update.

Fees & Travel Expenses

The cost of an initial evaluation is $600.00, while a program plan update will cost $400.00; each family will also be responsible for a prorated portion of our travel expenses (These additional costs incurred by Sara R. Johnson & Associates would traditionally be assumed by the family traveling to our Tucson, AZ clinic). Our intent in offering these "Traveling Clinics" is to provide the same services families would receive in our Tucson office, without their having to pay for the trip itself. Therefore, our travel expenses will be divided equally among all participating families. We do everything possible to keep these costs at a minimum, but we cannot predict how many families we will be seeing, the fluctuating cost of airfare, or varying costs of accommodations in different cities.

On occasion Sara Rosenfeld-Johnson will be providing initial muscle-based, speech, and/or feeding evaluations and program plan updates when she is speaking in specific locations. Her fee schedule is as follows: $800.00 initial evaluation, $500.00 program plan update.

Insurance

Please note that if you have insurance and your child has any history of feeding difficulties (gagging, reflux, choking, and food aspiration/pneumonia), or you have a plan that covers speech therapy, the evaluation or program plan update may be covered. We recommend including a letter from your doctor when your insurance paperwork is submitted. Please be mindful that it is the family's responsibility to contact their health provider, as we can only accept payment directly from the family (in the form of a personal check, Visa, MasterCard or American Express).

Scheduling

Once the appropriate dates for the Traveling Clinic have been determined, you will be asked to make a reservation. At that time, please send an e-mail with the days and times that work best for you. Normally we ask that you provide three options for your appointment times, numbered in order of preference. The therapist's itinerary will be based on a "first come, first served" process, so a timely answer would be most advantageous to you.

Cancellation Policy

Please note that we have implemented a "21-day cancellation notification" policy, which means we require, at a minimum, three weeks' notice for any cancellation. Since our goal is to keep costs down for the families, we make airline reservations approximately one month prior to travel so that we may take advantage of optimal travel fares. If you are interested in an evaluation or program plan update and would like to reserve a spot, you will need to provide us with a valid credit card, expiration date, and name as it appears on the card at the time of your reservation. This information should be faxed or called in, as e-mail is not typically a secure form of communication. Further, note that we will charge $150.00 to your credit card for late cancellations. This policy is implemented in an effort to avoid further financial burden to the families who honor their commitment and are forced to assume the additional cost resulting from last-minute withdrawal of a family from the schedule.
